NOTICE TO PATIENTS OF MEDCENTER PAIN MANAGEMENT CLINIC & ED CERDAY, MD:

MedCenter Pain Management Clinic in San Antonio has closed and Dr. Cerday’s last day of practice at this location was January 11, 2023.  Another physician will be taking over this location under a different clinic name as of January 12, 2023.

Your medical records will remain under storage with MedCenter Pain Management unless you choose to transfer your records to this new physician or to another physician of your choice.  Should you choose to continue your care with the new physician or another physician, you may complete a records release authorization available from MedCenter Pain Management to instruct MedCenter Pain Management where to send your medical records.  To request a records release authorization form, email admin@medcenterpain.com or request one in person from the staff at Worker’s Clinic/Dr. Beltran’s office.  

You will also receive via email a notice directly from Dr. Cerday with this information. Dr. Cerday appreciates being permitted to treat each and every one of you and wishes you the very best in your recovery.
